ORDERING INFORMATION
EyeKon Customer Service Department is open from 7:00 a.m. To 4:00
p.m. Eastern Standard Time (EST), Monday through Friday. Please
place lens orders by model number, diopter power and purchase order
number. No cash credit can be given for lenses returned that are
lost or damaged in shipment. EyeKon does not issue cash refunds.
All prices are F.O.B. Clearwater, Florida.
RETURNS
AND EXCHANGES
EyeKon
Medical accepts returned lenses for exchange only. Lenses being
returned MUST be accompanied by a RETURN AUTHORIZATION NUMBER from
the Customer Service Department.
The AUTHORIZATION NUMBER MUST be clearly displayed on the outside
of the shipping container. Returned merchandise should be shipped
by an insured, traceable method. No credit can be given for returned
lenses that are lost or damaged in shipment. EyeKon does
not issue cash refunds.
RESTERILIZATION
Do
NOT attempt to resterilize EyeKon Intraocular lenses. Return non-sterile
lenses to EyeKon for Resterilization. Obtain a "RETURN AUTHORIZATION
NUMBER" from the Customer Service Department and clearly mark
the container "FOR RESTERILIZATION".

- Constants shown are estimates. Values may be used as guidelines
in the calculation of implant power. EyeKon recommends surgeons
to develop their own values based on experience, technique, measuring
equipment and desired postoperative results.
